Fatality Review leads to a better understanding of how and why infants and children die and catalyzes actions to prevent other deaths. The National Center for Fatality Review and Prevention (NCFRP), in collaboration with state fatality review programs, developed and manages this web‐based reporting system. Users of this system can enter fatality review data so that their findings can be tabulated at the local, state and national level. Findings from these reviews guide program, service and policy efforts to keep infants and children safe, healthy and alive.

If you are not currently a registered user, please contact info@ncfrp.org for information on enrolling your state and local teams into the system.

The National Center is funded in part by Cooperative Agreement Number UG728482 from the U.S. Department of Health and Human Services (HHS), Health Resources and Services Administration (HRSA), Maternal and Child Health Bureau (MCHB) as part of an award totaling $5,149,996 annually with 0 percent financed with non-governmental sources.
